Female:
- Medical and Reproductive history evaluation
- Complete physical examination
- If > 45 years of age:
- Medical clearance from internist to include assessment of heart, lungs and kidney status
- Consultation with a high risk pregnancy specialist
- Uterine Cavity Study
- Sonohystogram or recent HSG (within 2 years)
- Trial of transfer
- Lab appointment for blood draw on day 2 or 3 of your menstrual cycle:
- FSH level on day 2 or 3 of cycle
- Estradiol Level on day 2 or 3 of cycle
- Other Blood tests:
- Blood Type and Rh factor screen
- Red blood cell antibody screen
- Hepatitis B serology
- Hepatitis C serology
- RPR serology
- HIV serology
Male:
- Medical and Reproductive history evaluation
- Semen analysis
- Semen wash
- Blood tests:
- Blood Type and Rh factor screen
- Red blood cell antibody screen
- Hepatitis B serology panel
- Hepatitis C serology panel
- RPR serology
- HIV serology
Genetic testing offered as appropriate (i.e. sickle cell disease, cystic fibrosis, tay sachs disease and others)
