Female:

• Medical and Reproductive history evaluation
• Complete physical examination
• If > 45 years of age:
  • Medical clearance from internist to include assessment of heart, lungs and kidney status
  • Consultation with a high risk pregnancy specialist
• Uterine Cavity Study
  • Sonohystogram or recent HSG (within 2 years)
• Trial of transfer
• Lab appointment for blood draw on day 2 or 3 of your menstrual cycle:
  • FSH level on day 2 or 3 of cycle
  • Estradiol Level on day 2 or 3 of cycle
• Other Blood tests:
  • Blood Type and Rh factor screen
  • Red blood cell antibody screen
  • Hepatitis B serology
  • Hepatitis C serology
  • RPR serology
  • HIV serology

Male:

• Medical and Reproductive history evaluation
• Semen analysis
• Semen wash
• Blood tests:
  • Blood Type and Rh factor screen
  • Red blood cell antibody screen
  • Hepatitis B serology panel
  • Hepatitis C serology panel
  • RPR serology
  • HIV serology

Genetic testing offered as appropriate (i.e. sickle cell disease, cystic fibrosis, tay sachs disease and others)
