Surgical evaluation and treatment allows the the physician to diagnose and correct problems in the uterus and pelvic cavity.

Hysteroscopy

This is an outpatient surgical procedure in which a fiber-optic tube is inserted into the inside of the uterus . This approach can be used to directly visualize the inside of the uterus (uterine cavity). Because it requires general anesthesia, laparoscopy is performed in a hospital setting.

After you are put to sleep, the cervix is dilated. Next, a fiber optic telescopic camera is passed through the cervix allowing for visualization of the uterine cavity. Once the camera is in place a manipulator attached to the camera can be used remove lesions or biopsy the lining.

Hysteroscopy is commonly used to remove certain lesions like polyps, uterine scar tissue and uterine fibroids.

Laproscopy

This is an outpatient surgical procedure in which a fiber-optic tube is inserted into the abdomen through a small incision in your belly button. This approach can be used to directly visualize the uterus, ovaries, tubes and other abdominal organs. Because it requires general anesthesia, laparoscopy is performed in a hospital setting.

After you are put to sleep, a small incision is made in your belly button. Through this, a hollow needle is passed and once the needle is within your abdomen, gas is passed through the needle . The gas lifts up your abdominal wall creating an “air pocket” into which a tube is passed. A fiber optic telescopic camera is then passed through the tube allowing for visualization of the abdominal contents. Once the camera is in place additional manipulators can be inserted through the abdominal wall. these manipulators are used to access the pelvic structures.

Laparoscopy is commonly used to remove certain ovarian cysts and treat other pelvic problems like endometriosis, scar tissue and uterine fibroids. However, with regards to tubal scarring, most cases are so severe that surgery aimed at tubal reconstruction does not improve pregnancy rates. In these cases, in vitro fertilization is most commonly recommended.
